How to report a concern

Use the contact page and include the page URL, the information at issue, the reason it may be incorrect, and an authoritative source when available. Do not include private medical records or graphic details.

Priority order

  • Incorrect crisis-contact information
  • Instructions that could affect immediate safety
  • Local resource closures or major eligibility changes
  • Incorrect credentials, review claims, or organizational status
  • Accessibility barriers and broken links
  • Other factual or editorial errors

Review process

The organization will compare the concern with primary official sources, consult a qualified reviewer when the change involves clinical or prevention guidance, and update or remove material when verification is incomplete.

Correction notes

Material changes to prevention guidance, crisis contacts, reviewer information, or organizational status should include an updated review date and, when helpful, a short correction note. Minor spelling or formatting changes may be corrected without a public note.
